The easiest way to get account approval (we recommend this for all customers) is to take 5 minutes and fill out our Wellness Questionnaire. This is entry level - what we call a 'Wellness Client' level. It is free and there are no strings attached.
It is simple to become a wellness client of Dr. Hale. The Wellness Questionnaire is a good place to begin. You may also may call us at 561-337-9435, choose option 2 to speak to Dr. Hale, or leave a message for Dr. Hale if he is not available. You can also email Dr. Hale directly at [email protected] You can also leave a chat message.
Once you register, Dr. Hale will send you an introductory email and you can correspond with him as needed. This is a complimentary service. Dr. Hale has 40 years of expertise and a very varied background in the wellness field. He can provide you with insight that you may find helpful.
The Wellness Questionnaire is optional (but is required) for professional brands. (This is by no means a medical history - it is 10 simple questions related to lifestyle, exercise and diet and your supplement regime or supplement needs. You only need to fill out what you feel is pertinent).
Lastly, all new customers are entitled to a free 10-15 minute phone consultation or the equivalent in email correspondence.
